This form must be completed at the end of your community shift.

IMPORTANT- CLIENT NOTE TIPS

• Shift commencement begins once you reach your first destination; be that at the clients address or the facility address, not from your home
• Shift completion ends either at the address of your last client or the facility address, not when you reach your home
• Kilometres are reimbursed for your travel between clients (if applicable) and when taking clients out as per shift directions
• Kilometres are not reimbursed to and from your home
• Community shifts have a set time based on funding that is provided to the client. If your shift is looking like it will exceed the allocated time you must call the Allocations team to get approval for extension of time

IMPORTANT INFORMATION FOR ORGANISATION

Please ensure you detail anything specific regarding your time with the client. These include but are not limited to:

• Variation in Shift Time
• Client Declines Care
• Traffic Congestion
• Additional Care Requirements

Any variation to the run sheet must be detailed. All information will be provided to the clients care provider.
